I once somehow manage to screw up the continuity on a channel for the whole day. Pre-recorded continuity was playing in the wrong places, i.e. an ident link in a menu, ECP link in an ident. There was a channel premiere on that day as well. And on the transmissions logs, Playout didn't notice.

For me, it would have to be the time I broke the SCART input attached to the TV which left me without sky for 2 weeks

The time when I did what I though was just a Freeview rescan but was actual a complete factory reset.
Which wouldn't have been a problem if a) the remote hadn't died so I was using the phone app - not any more once the reset wiped the WiFi settings - and b) Hitachi had given some way to get past the initial set up screens using the controls on the set itself.
This one wasn't mine, but I ended up dealing with the aftermath - ntl came out to fix an issue with my grandad's cable TV. They replaced the box but left the RF output on the default, which was co channel with BBC 2 from Emley, with the obvious results.
