I think the fire was at the end of the first series of Phoenix Nights, the second series ended with the Ted Robbins character being tricked into confessing starting the fire and Max and Paddy running off and into their Road To Nowhere.

The Blakes 7 ending was a classic with Avon smiling then cuts to the credits and the sound of gunfire. EXCELLENT STUFF!! We never saw Avon get shot so I don't believe he's dead.
