I thought +1 channels were supposed to be next to the originals, for example More4 and More4+1 are loads of numbers apart.

That's because originally on Sky Digital all channels were allocated a block of 3 numbers (except the terrestrial 5). Alot of boradcasters have used the second for a "+1" and then the third for a second channel (eg UKTV Gold, UKTV Gold +1, UKTV G2), if they then create a "+1" for the second channel it has to go in the next available slot on the EPG. The same happened to E4, E4+1 and More4.
They can't simply shuffle someone else further down the EPG just to fit in their new "+1" channel, as the other broadcaster would complain about being disadvantaged. Most broadcasters guard their EPG positions jealously (especially the ones near the top).

Media 247, in their new EPG listings page, have listed: EPG 123 - Player...is this a new channel launching by 28th Feb or a random channel moving from one category to entertainment?

"Player" is a spin-off from the programming block on Challenge of the same name. It will screen a mix of Challenge's poker programmes and some sports programming from Bravo. You can get more details on the link below...

Not from any of my usual reliable sources, so I could be wrong, but I've heard 03:15 mentioned for the channel number change.

EPG changes normally happen mid-morning, but with such a massive overhall they may trigger it in the wee small hours of Monday night/Tuesday morning to reduce disruption.
