It's quite clear the BBC as a corporation are at fault. It's just a question of who. Obviously Jonathan Ross and Russell Brand are at fault hence their suspension/resignation and apology but who else has some responsibility?

The "what" was obvious - its the "how" and to a lesser extent "why" that needed time to be investigated.
