Not necessarily, they just have to have a system checking there is output. Even a simple audio detector could trigger such a thing (which would be better than broadcasting videos without audio on a music channel).

Happens often.
I remember once in like 2002 on The Hits' first look, the Smash Hits graphics of the time kicked in one morning (except the logo DOG), and when they actually realised the channel ran completely DOG free for about half a day!
It usually happens inbetween DOG changes, i.e. when they're switching from selection to a chart or something.

That has actually happened once. During around early 2004 (using the gradient-style boxed graphics) it was a Classic Hits Weekend, and half way through a song it just went blank for like half a minute, then some old songs using the old graphics and selection numbers appeared, however without adverts and text at the bottom of the screen.
