No idea why they didn't just edit it out.
No one paid proper attention when preparing/ingesting the content ?
Presumably it was meant to be edited out but was missed, though of they were joining parts 1 and 2 together surely that is the only edit that was needed.
Unless if was intentional for some narrative reason
It's a deliberate addition but as it's having to badly cover up something that wasn't intended to be broadcast that way. If you look on iPlayer they have uploaded both the original separated out episodes and the 'joined' version that was broadcast on BBC One. Those slides have actually been created as replacement material in the combined one and it's clear from the unedited version why they were added.
As it was originally made to be two episodes, each episode had a very distinct point for closure and opening, with some pre-opening material too. So if you simply joined them together and just removed the closing titles, you'd have also had to change or cut several minutes of animation and re-edit some music, or the poor flow would have stood out even worse (like you get on some episodes of The Simpsons as aired in the UK when we don't have an extra break in like the US). Putting an 'end of' slide in place of one set of closing titles and a 'start of part' in place of the next one's original title slide seems to be their least-worst way of trying to get around a bad late decision.

The Sound Of Music Live a few years back used END OF PART going into a break, which just looked a bit bizarre.
I admit, as someone who lives in the area, I spent more time picking holes at the filming locations, like showing the wrong colliery and the ice rink looked nothing like the old Nottingham Ice Stadium. But I'm anal like that.

You’re not alone. In Broadchurch they used both Lyme Bay and Clevedon as filming locations, that led to inconsistencies with the direction of of sunsets and sunrises
