I guess whether that made it to the DVD would depend on whether it was added to the credits sequence in post production or transmitted from pres after the programme.
Sidenote - US: On the Buffy videos I have there are at least two dedications to dead colleagues, as well as an episode of Angel dedicated to Glenn Quinn, the actor who played Doyle.
Its not the sort of thing you'd add for a DVD, especially so for older programmes so I'd be inclined to argue that the death happened afer the filming of the programme (obviously) but before first transmission. Normally they're found between end of programme content and roll credits which says to me they're added in late-on post-production.
Our Director on The Price Is Right died 2 days before the end of filming, so our Christmas episode was dedicated to his memory and this was shown as part of the credits.

They dedicate films to actors when they die, an example being the US actor T J Walsh who died in 1998. There are 5 films dedicated to his memory:
Outside Ozona, The Negotiator, Dollar for the Dead, Pleasantville and Hidden Agenda, which was the last film he made.
They all would have been in various stages of post production when he passed on so the makers wanted to honour his contribution to their film. It's just like the plaque or memorial bench they put up outside a town hall when some renown councillor dies.

It's one of those things you know that are coming from about a mile off, usually a longer look at the credits or an extra slide come to mind after a long black pause.
ISTR when Big Ron out of Eastenders (he was a popular extra in the programme) died, at the end of the omnibus edition they faded out and then put up a caption saying Dedicated to The Memory... etc.

Or when Richard Whiteley died it was a totally different affair as they still had five episodes of Countdown to show, so Carol came on at the beginning of the first one to explain what had happened, on the same level she was really upset, and for the rest of the week the Channel 4 C/A always said: "These Shows were recorded before Richard fell ill, although they may be uncomfortable to watch, showing the episodes is what we believe Richard would have wanted" (or on the lines of anyway)

It was Des Lynam's first one, the caption read: "Dedicated to the Memory of Richard Whiteley 1943-2005".
In soaps when an actor past or present dies they usually show a photo and the caption 'John Doe 19?? - 2007' at the end of the next episode.
with new dvd releases I don't think they put them where people have died since production ceased, other wise a programme like Dad's Army would have dedcations to the entire cast, with the exception of Clive Dunn.
On the other hand a DVD of George and Mildred may have a dedication to Yootha Joyce as she died whilst the programme was still in production.
