Perhaps not quite the same as what you're talking about (continuity/pres rather than programmes themselves?) but a few years back the National Film Theatre - as part of their annual TV festival - used to put on an event where you could watch a news bulletin go out live on the cinema screen whilst listening to the gallery sound. After the programme the presenter, editor and director (IIRC) would then come to the NFT by taxi to discuss the bulletin that they'd just put out.

They don't seem to do it any more, unfortunately, but I went to see Channel 4 News and Five News in different years.

Obviously the people with the decision to make are the ones with the ability to record this sort of thing (or get hold of a copyright-free recording).

In terms of privacy it seems like it would be wrong to make public something like this is people were saying indiscreet or unprofessional things, or the recording seemed to cast blame upon an individual for something that went wrong.

But in the case of a smooth, seamless, troublefree, professional bit of broadcasting, I don't see there being major occasion to cause hurt or distress to anyone. However, I can see the point, and if it's not possible, it's just not possible.

No the decision would be that of the people featured and the management of teh station/programme involved.

Even with a 'smooth' programme there could be a lot of stuff that they wouldn't want you to hear and probably even more stuff they wouldn't want their rivals to hear!
