I didn't see it, but the version on Iplayer (http://www.bbc.co.uk/iplayer/page/item/b008m9t2.shtml) which is probably the BBC Two England version has the incorrect graphics but correct voice over.

Not as bad as what happened during
Buzzcocks
a few weeks ago - as well as the slug obscuring the score graphics, the ECP actually played out, with announcement,
before the credits had actually started

At the end of the day it's a highly automated computer system that controls the output. Like all computers it relies entirely on the data it's given. Sadly if the wrong in and out points for an end credit squeeze of IPP are put in, this kind of things goes to air. That said, they are supposed to be previewed of course ...!

You could just argue if the credits weren't squeezed at all then there wouldn't be any wrong in/out points to be input at all.
