It'd be interesting if ITV had an ad break going at that point. Either that or déjà vu.

About a year ago ITV1 was accidently fed to one of the iPlayer encoding chains instead of the network output, I think it was the BBC Three stream that got replaced with the Grand Prix for a while. I have not got the report to hand now, but IIRC the problem then was down to human error in CCA.
I'm pretty sure that ITV, 4 and 5 are available as audio sources in the radio studios at Bush, so a mistyped code into the Pro-Bel panel could result in the same thing happening. Although you would hope it would get spotted when the SM does a pre fade to set the levels.
Has this kind of thing ever happened before? More to the point - how the hell did it happen?

I remember a similar incident during Match of the Day a few years ago in the early days of the new playout centre. As I understand it, they move the channels between playout 'suites' late at night to bring them into the same playout suite and there's potential there for a switching error

Other channels such as ITV,4,5 Sky News & Sport, CNN, Euronews etc are all available on the SCAR router in the news centre, it's potentially very easy for any of them to be switched to anywhere.... but then every source is a wrong source except the one you want!

A similar problem on UTV last night (but not quite so bad!) During the football highlights a Teddington Studios VT clock appeared and the output then changes to a different programme which according to the Radio Times was Total Emergency which was airing on ITV1 at the time.

That was unedited footage of the Cricket World Cup being fed back to the Beeb and put out during Match of the Day in error.
There's been a couple on Sky Sports, specifically PremPlus when that was still going. I remember a Saturday 5.15 game where, early on, it switched to footage of a goal from one of the 3pm kick-offs (which hadn't been televised yet of course) being replayed several times from several different angles, presumably being edited for Football First. Also one Sunday afternoon match was replaced for a minute or two by what was presumably a Sky News crew waiting around by a doorway for someone to come out. Nobody watched PremPlus though, so nobody noticed.
