It may have been - the Sunday bulletin is self-opped (in Leeds) by the man himself

All mistakes are caused by either:
a) People;
b) Computers; or
c) A combination of the above.

In tv pres it's normally the last one - i.e. a computer throws a wobbly and, as no one's perfect, the operator may not deal with the problem quickly/well enough to prevent the viewer noticing.
