Yes, breakdowns used to be much more common. These days virtually everything is backed up, with two server copies running simultaneously, and a tape on a shelf as further backup if all else fails.
In the past there would have been one tape copy (or further back, film) running. If there was a problem with the machine it was in, it would have to be taken off air, loaded in another machine and put back on air.
OBs generally have a main and reserve line too, so it's also quite unusual to lose an OB unless there is a fault at the broadcast location feeding both lines.
