The glass blocks ident was done for BBC DVDs / Blu Ray to replace the 1997 ribbons ident. It exists constantly running under all channels in case everything else fails so there is always a picture of some sort.

Yes and no...

The 'Nations Clean' version of BBC One / Two play out the spinning blocks in place of junctions. At the end of a programme, it'll hold on the last item for three seconds after the end time (often the endcap or a fade to black), then cut up the spinning blocks until three seconds before the next programme - which will normally be black, but not always. (The previous version of the EastEnders titles would cut from the clock at 09:59:57:00 to three seconds of rippling River Thames before the zoom out started at 10:00:00:00.)

So the nations don't generally see the VT clock on items being played out from London?

God it really was a nauseating mess of an ident. Wonder if the moron who signed that one off was also responsible for the 3 hour long BBC 2 rebrand

When was this BBC 2 rebrand?

Ta-dah:

6am, 1st October 2008. All was not revealed afterwards as it was pulled before the next junction. From the snippets posted here, it was apparently even more hilarious than what eventually popped up in 2009.

I don't think Tai Chi Rotation is singularly quite so bad as to be placed on a level with the aborted BBC 2 refresh mess...

However, I would agree that the rotating motion - whether nausea-inducing or otherwise - is just a poor idea in itself. It doesn't look good.

If they'd had the idea during the shoot - starting in close on the reflection and simply move out from there. In a similar manner to the standard version's direction, and for that matter the majority of the set. Then that kind of consistency might have allowed it to fit in better. But this just was tinkering with the standard version... If it's not designed for the way in which it might be used, sometimes the best idea is to resist tinkering.
