Must have only affected SD then. Some comments on Digital Spy indicate Scotland was also unaffected. The lack of apology must be because the CA wasn't aware of the problem.

I think it's because it's so rare nowadays, thanks to the improved technology of TV playout - programmes played out through servers or discs rather than tape or film. (Someone with much more knowledge than me will correct me on that, I'm sure!).
Better for the broadcasters and the viewers… not so much for those of us who look forward to such things!
I think they're far less exciting nowadays when they do happen as well, which the Strictly sound issues seems to prove. Something like a recent one where BBC Two NI was switching between their own feed and the Network with no acknowledgement makes for pretty unentertaining viewing. It's only really live stuff that's at risk nowadays, like the Lambing Live fault (which seems to be its only claim to fame IMO) and the Something For The Weekend BST issues. The most "dramatic" is probably when last year's Stand Up To Cancer when catastrophically wrong during the late night repeat.
I'm always amazed at how excited posters get when there's a rare sound or picture glitch! Four pages of discussion about it! Can you imagine what it would be like if an old "Technical Issues" slide appeared? Who needs sex or a holiday when there's a blip during Strictly?

Yes, pre-recorded programmes tend to stay on once they're on air because there are two servers running in parallel, so if one has trouble they can go to the other one.

I remember about 10 years ago watching BBC Two, and it suddenly switched to BBC One, and then Back again to BBC Two, and then back again to BBC One, and then back finally to BBC Two. They had a voice over with an apology after apologising to analogue viewers.

I do remember Points of View in the nineties getting loads of letters because one week they managed to put the plugs in the wrong way round or something, so you could hear Food and Drink, which was on BBC2 at the time, faintly in the background of Points of View, and vice versa.
