Last night during Question Time on BBC One, it kept going to a pink screen, and at one point after it getting stuck on that screen, an animated programme menu cut into the programme, which leads me to believe they have adopted ITV1's method of dealing with technical faults (make sense seeing both were made by Red Bee).

Red Bee also designed the <media speak> complimentary graphics </media speak> for BBC2.
Its may have been a case of the menu was on hand whilst the breakdown caption was not. I could be completely wrong of course..

Possibly, but it seemed to animate in a different way to the normal menu. Could just be me not paying too much attention however.

It seems to me that STV's announcements are pre-recorded - there was a breakdown during Ant and Dec's Saturday Night Takeaway last year and they cut to a 15 year old breakdown tape with Scottish scenery with words superimposing over it. Not to mention they stretched it to 16:9.

The crew could be picked up quite clearly on the mics. Good to see no one sweared while they could still be heard when the This Morning caption was up
The crew can't be picked up on the mics, especially as the crew in the studio would be as quiet as possible. I assume all the feeds of the crews in the Gallery, Sound Dept, ect would be fed into that sound desk so it can then be fed to Phil and Ferns ear peices, so if the desk goes wrong like it did, they will end up on air.

The talkback from between gallery, crew and presenters will be a completely diffrent bit of infrastructure to that which handles programme sound. Talkback is normally within the sound department's domain, but it's very risky to have the 2 mixing technically for obvious reasons.
