I don't think ITV have a sombre ident, I think their still logo used during technical faults doubles up as the sombre ident. As for ITV using listings, it's been that way since 2003. I think the logic behind it is to advertise their shows rather than just saying 'Sorry, computer did it'.
What about slowing Urban Basketball down (including the music) so only the flowers are seen, or even use one of the more subtle breakbumpers, like the bike one.
the thing i like about the BBC compared to ITV is it's dignity. the bbc mostly are not afraid to put their hands up and say we f cked up compared to ITV unless they have a Real big cock up then they just show a scrappy programme menu and play jazzy music.

Programme slides still exist during the weekday education slot on BBC2 and during Sign Zone on BBC1, sure it's not everywhere but programme slides don't say as much as a breakdown slide can. Besides, them programme slides are a bit old hat now aren't they?
I think it would make sence to use a hybrid between a 'cockup slide' as I like to call it and a porogramme menu. That way it still informs the viewer that there are problems, but also shows what there is coming up.
