I always wondered what the systems at Downing Street and Westminster are, I assume the Westminster OBs are put through the studio there but how exactly does it work outside No. 10?

Arrow and worst of all, outside TVC - why not do it from the studio, after all it's inside the building behind you!
There's no easy way to get pictures from the car park to the news studios - so a live two-way from outside the building will use a news OB vehicle, either using a terrestrial link to the East Tower, or uplinked to a satellite and back down again...

I have read the details somewhere but can't quite remember off the top of my head - inject points tend to be cupboards with a couple of sockets for power, sound, and if you're lucky an ISDN line or two.
I think the Downing Street one goes via Millbank; some others in London go via Marylebone High Street. And the info sheet for the Piccadilly Circus inject point suggests that a back-watcher may be required..

are they BT RS1000 circuits? and if so do they have reverse video and audio for talk back?

i wonder how many inject points (away from BBC tv & local radio centres) there are in total across the country. i can think of nearly a dozen off the top of my head, there must be many more.

There was for a while a list of places in the UK with fibre connections installed by BT on the BT website, but it seems they've slimmed down their broadcast website and it's dissapeared. I think it was in the region of about 800 sites with fibre, and many many more with ISDN connections.
