Could be a technical issue then, couldn’t the CA have introduced reports if it was just a problem with the presenter. So it was basically normal just without a presenter in vision.
Not easy unless the bits of video are in the same place as the announcer. This would mean loading all the clips onto the playout system and then the director/announcer playing them in. Technically possible but in no way what the playout suite was designed for, or the directors trained for. I imagine it would cause a few issues with things like the EPG too.
Possibly with a bit of setting up and plugging in advance they could somehow send the announcers voice to a news studio and have them play in the bits. Not something I'd like to be faffing about trying to set up at 6am.
But then if the news studio is manned and operational then why not just cut the announcer out of the equation and get another voice to read themselves
IIRC during the 2008 Olympics (or some similar event) Breakfast moved to BBC2, which prevented many of the opts. On that occasion TC7 did a paper review to fill the time, but it has never been standard.
Presumably on the occasion Ballyboy mentions London developed a problem too close to an opt to get a feed from an alternative region (usually Southampton) so they ad-libbed a paper review to fill the time.

I suspect part of the procedure is to tell the replacement region NOT to opt, but stay on network, to avoid any comedy howl-round ?

Certainly a few years ago London output is not in the sustaining feed
