The automatic backup recordings made at BH are different to the recordings of breakfast, which may have been simply the RoT (Recording of Transmission) clipped up and re run.
Complete with breakfast clock. This is more than a sound issue surely.
Probably just a ‘dirty’ recording of the interview is all they could grab at short notice. The problem seemed to be the audio getting trapped into a feedback loop, sounded as if they were trapped in a very deep echo chamber
That suggests that a non-standard source of the recording was used I suspect. There is a permanent 'Breakfast Clean' feed available in NBH from Salford (which I believe is recorded on the standard production systems) to allow for Breakfast interviews to be turned round without graphics.
Not the first time that the wrong person was captioned in the weather bulletsin, happened earlier this year when Carol Kirkwood did the bulletin during Newsroom Live, one of them (the 11.30am one) captioned her as Simon King (by the 12pm weather bulletin, it properly said "Carol Kirkwood")

just looked at BBC News Channel / BBC World News schedule for next week it's just been updated to say World News Today in the 21:00GMT slot! But no BBC Four simulcast until 4 January!
Shaun Ley made his debut on Afternoon Live today, opening the program at 2 with "Hello everybody this is Afternoon Live with me Shaun Ley, standing in for Simon the real McCoy, which I guess makes me his decoy, today at 2..." . !

He was excellent as a stand in host in that slot, just like Martine Croxall. If Chris Eakin had still been around, he'd have been another.

Chris Eakin was brilliant last year when he hosted it for just 15 seconds with Maxine Mawhinney, when they returned to celebrate the channel's 20th birthday.
