Any Idea What This Classical Music Is?

I was wondering if anyone is able to identify the name and composer of the piece of classical music used at the end of this week's Casualty please? It appears around 46.00 in the episode and plays right up until the end.

I must also add it's a lovely and moving piece of music and worked well with the ending to make the close quite emotional.
