I do love a VT slate. I put them on everything, although the need to cue video to a count seldom comes up.

I'm off to my pop's house today, so I'll upload when I get home. Nightmarishly the pc I was using has chosen this weekend to turn itself off and not go back on, so you might get a different looking slate from an older template.

Pete has unlocked the "giant upload" facility at metropol towers, so you can have it as DV AVI, assuming you want to edit this into something.
