Were the "4:3 Title Safe Area" is located in the 2nd inner black line, thats the Title Safe Area for 4:3.
The red line, is the 16:9 Title Safe Area.

Why is the 4:3 title safe area made of five boxes, and the 16:9 title safe area just the one box?

would you need it for BBC World? IIRC the majority of World pres is 16:9 zoomed to 4:3 and only the news itself is in 14:9, in which case it should be designed for 16:9 with 4:3 safezones respected.
I'm not saying its not a useful addition as those safe areas need to be kept in mind but graphics are meant to fit inside 4:3
